This form must be completed in its entirety and presented in person at Wonderland Sydney's reception office (Monday to Friday, 9am-2pm) together with your Wonderpass membership card for identification purposes, by no later than Monday, 31 May 2004.

Once Wonderland Sydney has received your completed Pro-Rata Refund Form you will be issued with an acknowledgment of receipt. Your form will then be processed from Tuesday, 27 April 2004, the outstanding value of your Wonderpass will be refunded from this date based on your purchase price.
